How to create a form on Uniqode

Uniqode Forms can be used for surveys, customer feedback on products or events, leads, and more.

Uniqode Lite plan or above

Creating a Form on Uniqode

  1. Sign in to your Uniqode account

  2. On the Overview page, click on Build Forms

    Alternately, you can also head to Forms under Campaigns on the left panel and select +Create Form.

  3. Name your form to start creating it

  4. Select from several pre-defined form templates or create your own from scratch.

  5. Click on Add a Question to start adding questions

  6. You can choose from our list of question types. You can hover over each question type to see a preview and description.

  7. Add details of your question by clicking on the question type

  8. Click Add to add it to your form

  9. Once you have added the questions you need, click Next to customize your form

  10. You can choose from Public Themes or add your own

  11. Once you are happy with how your Form looks, click Next to set up your form

  12. You can edit your Form Name, enable Email Notifications for form responses, add a Facebook pixel ID or Google Adwords ID for retargeting and set the default language for your form.

  13. Click Finish to complete setting up your Form.

Voila! Your form is ready and will appear on the list of other forms under your account.

Adding details to your question

Once you choose your question type, here is how you can add details

  • Enter your Question/Title.

  • Provide a brief Description of your question.

  • Optionally, you can add an image to help the user understand your question better.

  • To make a question mandatory, toggle on Required.

You can add a logic jump to create a pathway for users to follow based on their responses. To do so, click Logic Jump on the right of each question. You can redirect users to specific questions based on their responses or skip them directly to the last question.

You can also track each user's score based on their responses with the Score Calculator. This is ideal for form-based quizzes.
