Uniqode enables you to create QR Code-based forms to interact with users and collect responses. They can be used for surveys, customer feedback on products or events, leads, and more.
Note: Forms are available on Lite and above plans only.
Creating a QR Code-based form on Uniqode
Step 1: Select Form
To start, log in to the dashboard and select Form under +Create.
You can also create a new Form by heading to Forms under Campaigns on the left panel and selecting +Create Form.
Step 2: Select your form template
Start by giving your form a name.
Pick from several pre-defined form templates or create your own from scratch. For the purpose of this tutorial, we will create a new form from scratch.
Step 3: Setup the form
It's time to start creating your form.
In the Create tab, define your question as follows:
Choose the Type of response you want from the user, such as long or short form, yes/no answer, etc.
To make this question mandatory, toggle on Required.
Enter your Question.
Provide a brief Description of your question.
Optionally, you can add an image to help the user understand your statement better.
Once done, click Add To Form to add the question to your form. Similarly, add all your questions to the form.
You can add a logic jump to create a pathway for users to follow based on their responses. To do so, click Logic Jump on the right of each question. You can redirect users to specific questions based on their responses or skip them directly to the Thank you screen.
You can also track each user's score based on their responses. This is ideal for form-based quizzes. To do so, click More Options in the right corner of each question and click Calculator.
Step 4: Design your Form
Click Next to move to Design. Here, you can change the theme and layout of the form.
Step 5: Customize your Form
Click Next to customize your Form before finishing setting it up. Here, you can edit the Form's name, add a Facebook pixel ID or Google Adwords ID for retargeting, set the default language, and enable email notifications to notify you or the respondent when the form is filled.

Click Finish to complete setting up your Form.
Voila! Your form is ready and will appear on the list of other forms under your account.
Connect your Form to a QR Code
Step 1: Create a QR Code campaign for your Form
Now that you have created a Form, you can connect it to a QR Code. Head to the Overview page, click +Create and choose QR Code.
Step 2: Setup the QR Code for your Form
Choose your Form from a dropdown menu. Select Create New Form if you still wish to create a new form.
Step 3: Customize the QR Code for your Form
Now that you've set up your form, it's time to customize the QR Code. You can edit the QR Code's pattern, frame, eyes, and color.

Once you're done customizing the QR Code, you can download it for deployment in one of several formats available.
Step 4: Collect responses
When a user completes your form, their response is securely recorded and stored in your account.
You can view all your form responses under the forms section to analyze valuable feedback and gain insights.
Note: Each plan comes with a set number of responses you can collect (see table below).

If your needs exceed the included response limit, you can easily purchase additional response packs for your chosen plan. This ensures you can continue to collect valuable feedback without interruption.
Download Form responses
To download responses from your form:
Step 1: Go to Forms
Click on Forms under Campaigns on the left panel of your dashboard.
Step 2: View all responses
Select the form for which you want to download responses and click on "Responses" to view all the responses that have been submitted for your form.
Step 3: Download responses
Select the format (CSV, Excel, or PDF) to download the responses and click "Export".
The responses will be downloaded to your device in the selected format. You can then open the downloaded file and view the responses.
